Lipo-C has been discussed in the ExcelMale community since 2016. Over nearly a decade, hundreds of members have shared dosing protocols, results, and frustrations. This guide consolidates that community experience alongside current clinical protocols to give you a clear, usable answer to the core question: how much, how often, and does it work?
Short answer: Lipo-C is a supportive metabolic therapy, not a primary fat-loss drug. It works best as an adjunct — particularly alongside GLP-1 agonists, TRT, or caloric restriction. Here's what the evidence and community experience say about getting the dosing right.
| WHAT CHANGED IN 2026 The biggest shift in Lipo-C use is GLP-1 co-prescription. Semaglutide and tirzepatide can accelerate muscle loss and impair liver fat clearance during aggressive caloric restriction. Lipo-C is now commonly prescribed alongside GLP-1s to mitigate those effects. |
1. What Is Lipo-C?
Lipo-C is a compounded injectable formulation combining lipotropic agents (compounds that facilitate fat breakdown) with B vitamins and amino acids that support liver function and energy metabolism. It requires a prescription and is manufactured by compounding pharmacies including Empower Pharmacy."Lipotropic" means fat-moving. These compounds prevent fat from accumulating in the liver, support bile production, and improve the efficiency with which the body converts stored fat to energy.
Lipo-B vs. Lipo-C: Lipo-B contains the core MIC complex (methionine, inositol, choline) plus B12. Lipo-C adds L-carnitine — the critical fatty acid transporter — plus vitamins B1, B5, and B6. Lipo-C is the broader-spectrum formulation and the more commonly prescribed version for metabolic support.
2. Ingredients & What Each Does
Standard Lipo-C formulation (Empower Pharmacy): Methionine 15 mg/mL / Choline 50 mg/mL / L-Carnitine 50 mg/mL / Dexpanthenol (B5) 5 mg/mL in water for injection. Some compounders add inositol, B12, thiamine, or pyridoxine.| Ingredient | Conc. | Role |
| L-Carnitine | 50 mg/mL | Shuttles long-chain fatty acids into mitochondria for oxidation. Most evidence-backed component for metabolic support. |
| Choline Chloride | 50 mg/mL | Essential for phosphatidylcholine synthesis and bile production. Prevents fat accumulation in the liver. |
| Methionine | 15 mg/mL | Sulfur-containing amino acid; methyl donor for SAMe synthesis. Supports fat export from hepatocytes and detox pathways. |
| Dexpanthenol (B5) | 5 mg/mL | Converted to coenzyme A, required for fat and carbohydrate metabolism. Reduces injection site inflammation in some formulations. |
| Inositol | Varies | Insulin-sensitizing; supports liver fat metabolism. Often included in MIC formulas. |
| Cyanocobalamin (B12) | Varies | Supports red blood cell production, methylation, and neurological function. Often the primary driver of the energy effect users report. |
| COMPOUNDING VARIABILITY Lipo-C formulations are not standardized across compounders. Concentrations, added vitamins, and beyond-use dates vary. Always confirm the exact mg/mL composition with your pharmacy before setting a dose. |
3. Dosing Protocols: The Full Table
There is no FDA-approved dose for Lipo-C. The protocols below are drawn from Empower Pharmacy, Defy Medical, clinical spa protocols, and ExcelMale community experience.| Protocol | Dose | Frequency | Route | Best For |
| Starting / Tolerance Test | 0.5 mL | 1–2×/week | SQ or IM | First-timers; GI-sensitive patients |
| Standard Maintenance | 1 mL | 2–3×/week | SQ or IM | Most patients; ongoing metabolic support |
| Daily Subcutaneous | 1 mL | Daily (AM) | SQ | Users prioritizing energy and consistent carnitine levels |
| High-Dose IM | 2–3 mL | 2×/week | IM only | Clinical spa protocols; aggressive fat loss phases |
| GLP-1 Adjunct | 1 mL | 2–3×/week | SQ or IM | Co-use with semaglutide or tirzepatide for muscle preservation |
| Weekly Maintenance | 1–2 mL | 1×/week | IM | Long-term users; cost-conscious maintenance |
| COMMUNITY CONSENSUS STARTING POINT Based on years of ExcelMale community reports, the most reported starting protocol is 1 mL subcutaneously or IM, 2–3 times per week. Most members who see subjective benefits report noticing them within 2–4 weeks. If no effect at 3 weeks, consider increasing to 2 mL or moving to daily 1 mL before concluding it doesn't work. |
Titration Guidelines
• Weeks 1–2: Start at 0.5–1 mL, 1–2×/week. Assess tolerance (odor, injection site reactions, GI changes).• Weeks 3–6: Increase to 1 mL, 2–3×/week if tolerated. This is the sweet spot for most users.
• Week 6+: If energy or fat loss response is minimal, consider 2 mL IM 2×/week, or daily 1 mL SQ.
• Reassess at 12–16 weeks: If no measurable metabolic or subjective benefit, reassess whether Lipo-C is the right tool.
4. How to Inject: Sites, Needles, Technique
Subcutaneous (SQ) Injection
Use a 1/2-inch insulin syringe (28–31g). Best sites: pinched abdominal fat (belly), outer thigh. Inject at a 45-degree angle into the pinched fold. Keep volumes at or below 1 mL per SQ site — larger volumes cause firm, slow-absorbing lumps.Intramuscular (IM) Injection
Use a 5/8-inch, 25–26g needle. Best sites: vastus lateralis (outer thigh), ventroglute, or dorsoglute. IM is preferred for volumes above 1 mL and for faster absorption. Draw back slightly to confirm you are not in a vessel before injecting.| THE B-VITAMIN ODOR ISSUE B12 and other B vitamins in Lipo-C create a distinctive sulfur or vitamin smell that can emanate from sweat, urine, or even breath within hours of injection. This is harmless but worth knowing. Frequency and intensity vary by formulation and individual metabolism. |
5. Stacking: B12, GLP-1s, and TRT
Lipo-C + Vitamin B12
Some compounders add B12 directly to the formula; others provide it separately. If your Lipo-C does not include B12, a separate B12 injection 1–2×/week is worth considering for energy support. Many ExcelMale members report that B12 is the component they feel most acutely.Lipo-C + GLP-1 Agonists (Semaglutide / Tirzepatide)
This is the fastest-growing use case in 2025. GLP-1 receptor agonists drive aggressive caloric restriction, which accelerates lean muscle loss and can impair liver fat clearance. Lipo-C is being co-prescribed to:• Maintain L-carnitine levels during aggressive caloric restriction
• Support hepatic fat mobilization as the liver processes increased lipid flux
• Partially offset GLP-1-induced fatigue via B-vitamin and carnitine support
No pharmacokinetic conflict exists between Lipo-C and GLP-1 agonists. They are typically injected at different times of day. Always confirm this combination with your prescribing provider.
| "Started semaglutide at 1 mg/week and felt completely wiped out by week 3. My doc added Lipo-C 1 mL 3x/week and the energy crash improved noticeably within 2 weeks. Not sure which component is doing the work but something is." — ExcelMale community member, 2024 |
Lipo-C + TRT
Testosterone replacement therapy improves insulin sensitivity and lipid metabolism but does not directly support hepatic fat clearance. For men on TRT who are also in a fat loss phase, Lipo-C can provide complementary support. L-carnitine in particular has independent literature support for improving lipid profiles in men on TRT.6. Side Effects & What to Watch
| STORAGE NOTE Lipo-C should be refrigerated at 36–46°F. Most compounded vials have a 30-day beyond-use date after dispensing. Keep away from light. Do not freeze. |
7. Does Lipo-C Actually Work?
Honest answer: it depends on what you're asking it to do.Each individual component — L-carnitine, choline, methionine — has legitimate mechanistic data supporting fat metabolism and liver function. The question is whether injectable co-administration produces clinically meaningful effects on body composition in humans who are already eating adequately.
What the Community Reports
| "It's not a magic fat burner. But when I'm dieting hard and training, it genuinely seems to blunt the fatigue and help me recover faster. I've run it on and off for two years. Keep coming back to it." — ExcelMale forum member, 3+ years on TRT |
8. Frequently Asked Questions
How much Lipo-C should I inject?
Most protocols start at 0.5–1 mL injected subcutaneously or intramuscularly 1–3 times per week. Dose can be titrated up to 2 mL per injection depending on tolerance and goals. Do not exceed 3 mL per single subcutaneous injection due to lump formation and absorption issues. Most users find the sweet spot at 1 mL, 2–3×/week.
Can I stack Lipo-C with semaglutide or tirzepatide?
Yes, and this is increasingly common in 2025. Lipo-C is used to support liver health, maintain L-carnitine levels, and reduce GLP-1-associated energy crashes during aggressive caloric restriction. There is no known pharmacokinetic conflict. Always confirm this combination with your prescribing provider.
How often should I inject Lipo-C?
Most clinical protocols recommend 2–3 times per week. Some high-frequency protocols use daily 1 mL subcutaneous injections. Weekly injections are used for maintenance. The right frequency depends on goals, cost tolerance, and individual metabolic response. Start at 2×/week before increasing.
Does Lipo-C actually work for fat loss?
It works as a supportive therapy, not a primary fat-loss intervention. The components each have documented roles in fat metabolism and liver function. Community reports show best results when Lipo-C is combined with a caloric deficit and resistance training, or alongside GLP-1 therapy. Standalone fat loss results are modest at best.
What is the difference between Lipo-B and Lipo-C?
Lipo-B contains the core MIC complex (methionine, inositol, choline) plus B12. Lipo-C adds L-carnitine — the fatty acid transporter that shuttles fats into mitochondria for energy production — plus additional B vitamins (B1, B5, B6). Lipo-C is the broader-spectrum formulation.
Where do you inject Lipo-C?
Lipo-C can be injected subcutaneously (belly fat, outer thigh) using a 1/2-inch insulin syringe, or intramuscularly (glutes, vastus lateralis) using a 5/8-inch 25–26g needle. For volumes above 1 mL, IM is preferred to avoid subcutaneous lumps.
How long does a 30 mL vial of Lipo-C last?
At 1 mL three times per week, a 30 mL vial lasts approximately 10 weeks. At daily 1 mL injections, it lasts about 4 weeks. Most compounded vials have a 30-day beyond-use date after dispensing. Typical cost is $76–100 per 30 mL vial.

Hey Folks,
I'm curious to see who is using Lipo-C, what your dose/injection schedule is, and what your results (subjective or not) are so far.
I recently started with Defy. Dr. Saya had no problem adding Lipo-C to my regimen. (I'm dying for more energy, and also want to lose fat / maintain muscle as I diet to get back in shape).
It appears that there's a lot of flexibility in terms of how much and how often to inject Lipo-C, with the only downside to more and more often being higher cost. I've been guided to start with 1ml 2-3x per week. But the Defy website mentions a high end of 1ml every day! (see below)
How to Administer Lipo-C:
Using a 5/8" 25g or 26g needle, inject 1ml-3ml of Lipo-C twice per week.
As an alternative, Lipo-C can be injected daily. Using a 1/2" "insulin" syringe, inject 1ml of Lipo-C subcutaneously every morning.
Money is not an issue for me right now - and I'm thinking about 1ml EOD or something for the first month to kind of "kick start" things. But wanted to get your feedback first.
